Organisations outside the NHS provide health services on behalf of your GP surgery, hospital and NHS England. These include services you can access using the NHS App or the NHS website. We call these third party (connected) services.
The local and national services available to you depend upon:
- the organisations chosen by your GP surgery
- the integrated care board (ICB) your surgery belongs to
- those provided or chosen by NHS England
For those chosen by your GP surgery (local services), we show the names of organisations in your NHS App or the NHS website before you use their service. If you're still unsure who they are, ask your GP surgery.
Check your COVID-19 vaccine record
The Check your COVID-19 vaccine record service lets you view your vaccination details, like the name and batch number, and report any side effects you have experienced.
This is a national service provided by NHS England.
We only support queries about how to access this service in your NHS App, not queries about the service itself.

Personal health records
A personal health record stores information about your health and care online. It's managed by yourself, and health professionals can add information to it.
We can only support queries about Patients Know Best Ltd and Substrakt Health relating to their services in your NHS account.

Contact your GP surgery
You can contact your GP surgery using the NHS App or by logging into the NHS website. If it's available you may be able to:
- contact your GP surgery about a health problem
- contact your GP surgery for a document or update
