   I have been de-registeted from the practice for cancelling an appointment, within 24 hours. I have apparently done this 3 times in the last five years. I do not recall seeing or signing anything outlining this and have been forced into using a private practice alongside for over a year due to the ridiculous wait time for an appointment.
