  1. Review titled It takes Welbeck 1 week to complete a Medical Certificate of Cause of Death

by Anonymous - Posted on 23 January 2025

My mum was a patient at Welbeck Surgery for over 25 years & recently passed away from cancer, a condition well known to the surgery with an expected outcome. On 9th January, I informed the surgery of her passing to initiate the process of obtaining the necessary documents for her funeral. Based on average time to complete, the process should have taken 4-5 days, as follows: 1.GP completes the MCCD: with the NHS recommending prompt action. 2.Medical Examiner reviews and confirms the MCCD: Avg. 1-2 days. 3.Registrar is informed, and an appointment is booked: same day or next. 4.Death Certificate and Green Form issued, allowing funeral arrangements. Despite these expectations, Welbeck Surgery took 7.5 days just to complete the MCCD, which shows a lack of prompt action. When I followed up on 13th January, I was informed that no action had been taken. On 15th January, I called again & encountered a receptionist who just wanted to talk over me & seemed defensive about my follow-up. The Medical Examiner only received the MCCD at 14:44 on 16th January, over 7.5 days after I first notified the surgery. However, once they received it, the entire process—from medical examiner review to issuing the death certificate & Green Form—took only 3 days, highlighting the inefficiency of the GP surgery. Additionally, despite my request for myself to be contacted instead of the next of kin due to personal reasons, this request was ignored, adding further stress to an already difficult time. The extended delay caused heightened emotional distress, knowing my mum was lying in the mortuary instead of the funeral home, where we could have visited her. The lack of communication, urgency, & empathy from Welbeck Surgery made this experience even more painful. It is deeply disheartening to feel that after 25 years of being a patient, my mum was treated as nothing more than a number, with the surgery's approach changing the moment my mum was no longer an active patient, with no consideration for the emotional impact on the family. I am extremely disappointed with the handling of this situation & the poor attitude of staff. This is not a scenario where an apology or condolences would even come close to addressing the matter. I do understand that delays can happen, but 7.5 days is more than a delay & is in no way ‘prompt’ as the NHS suggests. A guarantee/pledge from the practice managers that no other family will have their trauma exacerbated by Welbeck’s poor communication, attitude, & dragging of heels is what is called for. Given the Surgery’s mission statement, vision, & values to provide a compassionate, empathetic service based on excellence, they have failed in every aspect in this instance. I feel like the employees, at every level, should be reminded of their pledge on a daily basis, as these mantras are clearly not at the forefront of the service they are providing. If only they were, our treatment may have been much better & this review would not exist.

I want to express my deepest condolences on the passing of your mother. I understand that this is an incredibly difficult time for you and your family, and I am truly sorry for any additional distress caused by the delay in processing her MCCD. We deeply regret that we did not handle this as swiftly as we should have, and sincerely apologise for the upset this has caused. Please be assured that we are reviewing our procedures to ensure that this does not happen again in the future and will be speaking to all staff to improve patient communication and to reiterate our values. We agree that your experience was not acceptable. As you have posted this anonymously I will respect your privacy and not contact you, but please do not hesitate to get in touch if you would like to discuss this further, or if there is anything else we can do to support you. I am truly sorry for our part in adding to what is already a painful time for you. Sarah Childs - Practice Manager

Asked to make a GP appointment and on two occasions was assigned an advanced nurse instead of a doctor without being informed. Thr care provided by the nurse substitute was not of good quality.

Thank you for your comments. Every patient who makes an appointment should be clear who they are booked in with. Our reception team have been reminded of this and will endeavour to ensure this. We are also putting in place other ways for patients to identify different staff groups, such as clear information in the waiting area of the varied roles within Primary Care. If you wish to discuss your comments further please don't hesitate to contact me.

I went to see the ANP this morning for a healthcare review. So professional, approachable and kind. I have always found the staff at the surgery so lovely. It seems like a well run surgery despite the current NHS climate.

I was unwell while visiting the practice and the staff showed real compassion in helping me settle and making sure I was ok. They also supported me in steps to take prior to and following my appointment to aid my health. Throughout my interactions with staff I have been treat well and have felt cared for.
