Referral method
Contact your GP to request an appointment for a NHS Health Check. Free NHS Health Checks are available 5 yearly to residents aged 40-74.

Please note if you are already receiving treatment for one of the following health issues you may not qualify. These include, diabetes, kidney disease, heart problems, high blood pressure or raised cholesterol. Please speak to your GP to check.
Description of service
The NHS Health Check is a simple check of your heart health. Aimed at adults in England aged 40 to 74, it checks your vascular or circulatory health and works out your risk of developing some of the most disabling – but preventable – illnesses. It is free of charge, including any follow-up tests or appointments.
