About

The Community Learning Disability Team offers support and advice to adults with learning disabilities, their families and carers in the Croydon area. Services offered include Physiotherapists, Psychologists, Nursing, Dietetics, Health Facilitation, Acute Liaison Nursing at hospital sites and Speech and Language therapists.

Target group

• People with formal diagnosis of a learning disability
• have specialist health needs their GP and mainstream services cannot help with
• live in Croydon and registered with a Croydon GP
• over the age of 18.

Referral method

You can telephone, send a letter, email or visit. Staff will then carry out an assessment of your needs.
