About

The Downham Elderly Health Project is an area based service which develops and delivers preventative services and activities to help to support older people living in the Downham, Bellingham and Grove Park Lewisham wards.

The project can help support older people to manage their long-term conditions, improve their quality of life and delay or prevent the need for more intensive interventions.

The programme can respond to the needs of older people and carers who access the service by, for example, supporting the older person to access groups or services available locally, or by support to access other statutory or non-statutory sector agencies

Opening times

8.30am - 5.30pm Monday - Friday.

Access

Groups are held at various locations all of which have disabled access.

Main office base is accessible.

Catchment area

Lewisham

Target group

Anyone over the age of 55 living in the South East of Lewisham.

Referral method

Open referral process, self referrals very welcome.
