Therefore patients, their families and/or carer may access this service. The advisers can offer assistance with benefit checks, form filling, grant/charity applications and appeals including tribunal representation. The service is delivered by telephone, email, letter or fax and if necessary within their homes or in a healthcare setting. The service also offers outreach services within hospitals and other health care settings throughout County Durham.
