This service is available to eligible residents or patients registered to a GP in this catchment area.

Referral method

If you are registered with a GP in Halton you should receive an invitation for an NHS Health Check once every five years. If you receive your invitation letter contact your GP to arrange an appointment. Alternatively you can contact Halton Health Improvement Team.

Description of service

The NHS Health Check is a simple check of your heart health. Aimed at adults in England aged 40 to 74, it checks your vascular or circulatory health and works out your risk of developing some of the most disabling – but preventable – illnesses. It is free of charge, including any follow-up tests or appointments.

Service type

NHS Health Checks are available at GP practices across Halton. Halton Health Improvement Team also offer eligible residents NHS Health Checks at community venues – contact us for more information on 0300 029 0029.
