About

Providing Welfare & Benefit advice and support with completion of application forms to patients across Lincolnshire who have been diagnosed with a life limiting illness such as Cancer, Parkinson’s/Dementia & Alzheimer’s/Motor Neurone Disease/Heart Failure to name a few and are registered with a Lincolnshire GP.

Monday – Thursday 8.30-4.30pm & Fridays 8.30-4pm

Across the whole of Lincolnshire – registered with a Lincolnshire GP

Target group

Anyone diagnosed with a life limiting illness including Cancer, Parkinson’s/Dementia & Alzheimer’s/Motor Neurone Disease/Heart Failure to name a few. Supporting their family members and Carers as well.

Referral method

Referrals can be made by anyone including self referrals, on our Welfare & Benefits Referral line 01476 513544
