Service details
We offer a series of structured one to one appointments with a named key worker which are designed to offer support towards goals agreed with the young person.
We use the outcome Teen Star to help develop a personalised support and recovery plan for everyone. To make sure we provide each person with the best comprehensive service we work closely with other young people services.
If a young person has a case manager in another young peoples’ service we will work closely with them to ensure a joined up package of support.

Monday to Friday 9am - 5pm

Referral method
Self referral or via other agency. The majority of our professional referrals come from the Prevention and Education Team, the Youth and Family Support Service and the Youth Offending Team
General notes
Provides support to family and friends via the Changes service.
